Population Health & Clinical Education
HPN offers clinical educational sessions for all of our providers, clinicians, pharmacists, and nurses — on clinical guidelines, utilization management, and chronic disease states. We also focus our sessions on performance measurement, quality of care, and education on 5 Stars and HEDIS. The level of education is tailored to the specialty and level of practice, and all materials are based on the most current national guidelines.
Our Population Health Management and Preventive Medicine departments are the cornerstones for these educational sessions, which demonstrate the quality of care we offer to our patients. HPN is in the process of providing CME for these sessions.
